In order to facilitate the sale of my unit and pursuant to the provisions of Section 11-135 of the Maryland Condominium Act and Sec 11-B of the Maryland HOA Act., I hereby request that you furnish me the information required by the referenced section concerning the condition of the Association and the Unit named above, through preparation of a "Certificate for Resale."

I understand that preparation of the Certificate for Resale must be completed within twenty (20) days from the receipt of this request, and that payment in full for preparation of the Certificate must accompany this request.

Enclosed is a cashier’s check or money order in the amount of $150.00 payable to Potomac Valley Management, LLC for preparation of the required Certificate for Resale. Packet will be complete within 15 days. If packet is needed prior to, please add an additional $50.00 rush fee for processing within 7 days of receipt.

I hereby certify that any improvements or alterations made to the unit, or the limited common elements (if any), appurtenant thereto are not in violation of the Association instruments.
